Community Health Workers (CHW) promote the health and wellbeing of Latino families in our region.

 

PROGRAM OVERVIEW

 
 

LCC offers a Community Health Worker (or “Promotor” in Spanish) model aimed to promote the health and wellbeing of Latino families in Pittsburgh. The program connects the community with resources to ensure that families have what they need to create healthy environments.
LCC’s Promotores live and work in the Latino community in Pittsburgh, ensuring that all Latinos have equitable access to services and the tools to implement a healthy lifestyle. Most importantly, our community health workers work to reduce the isolation many Latinos experience in our region.

HOME VISITING

The Healthy Families Program - or “Familias Saludables” in Spanish - is LCC’s home visiting program that supports parents in the prenatal stage through the time their child turns 2-years -old. 

Promotores provide services including:

  • Connecting families to appropriate services and resources, such as the health care system, public benefits, and basic needs.

  • Screening children for developmental delays using Ages & Stages Questionnaires (ASQ), and increasing access to early intervention.

  • Providing information and guidance on a wide range of topics, including breastfeeding, nutrition, safe sleep, injury prevention, etc.

  • Promoting positive parenting skills and modeling positive parent-child interactions.

 
 
 

Healthcare Navigator Program

Families with children under 18 are eligible for this program, in which Promotores help families access affordable, quality healthcare services. The goals of the program are to:

  • Connect families with primary care services, specialist care, and social services. Promotores help families find low-cost clinics, make special care arrangements, and fill out applications like SNAP, WIC, or Medical Assistance.

  • Identify and resolve families’ barriers to care.
